Therapists

Kelly Mitchell – MS, PT, LAT, ATC
Manager of Rehabilitation Services
Physical therapist and a certified athletic trainer. She received her bachelor’s
degree in athletic training and her master’s degree in physical therapy from the University
of Indianapolis. She has also received advanced training in manual therapy
and functional capacity evaluations. Kelly has experience in pediatric rehabilitation
and has a special interest in rehabilitation of spinal injuries.

Jay Slusher – MS, PT
Physical therapist. Jay brings a wide variety of knowledge and over 15 years of
experience to SportWorks. After receiving his bachelor’s and master’s degrees in
microbiology from Ball State University he obtained a master’s in physical therapy
from the University of Indianapolis. Jay has extensive training in various manual
therapy techniques and functional capacity evaluations.

John Anderson – DPT, LAT, ATC
Physical therapist and certified athletic trainer. He received his bachelor’s
degree in athletic training from Anderson University and his doctorate in physical
therapy from the University of Indianapolis. He specializes in knee and shoulder
treatments.

Tyler Smith – DPT, CSCS
Physical therapist. He received his bachelor’s degree in human biology and his
associate’s degree in business administration from the University of Indianapolis.
He also obtained his doctorate in physical therapy and is a certified strength and
conditioning specialist.

Jeff Waldo – PTA
Physical therapist assistant. He received his physical therapy assistant associate’s
degree from the Community College of the U.S. Air Force and his bachelor’s
degree in business administration from Regis University, Denver, Colorado. Jeff is
retired from the United States Air Force and brings 25 years of experience in outpatient
orthopaedics and sports medicine.

Nancy Ewing – OTR, CEAS
Occupational therapist. She received her bachelor’s degree in occupational therapy
at Western Michigan University. She specializes in upper extremity rehabilitation.
She has over 15 years experience in industrial rehab and is also a certified
ergonomic assessment specialist.

Athletic Trainers
|
Tara Humphreys – LAT, ATC
Athletic trainer. Tara received her bachelor’s degree in athletic training from Indiana
Wesleyan University. She provides sports coverage for Southwestern and
Waldron High Schools.

Amy Pence – LAT, ATC
Athletic trainer. Amy received her bachelor’s degree in athletic training from Anderson
University. She provides sports coverage to Shelbyville High School.

LaKisha Jones – LAT, ATC
Athletic trainer. LaKisha received her bachelor of arts degree in athletic training
from Franklin College. She then completed her master’s of science degree with a
concentration in athletic training from the University of South Carolina. She provides
sports coverage for Triton Central High School and Morristown High School.
