We have a hassle-free referral process. Your patient can contact us directly to request an evaluation,
or your office may contact us with your fax number and we’ll fax over a brief referral form.
We provide services as fee-for-service only, but we will gladly assist patients in confirming out-ofnetwork
benefits for a surgical evaluation.
We accept assessment referrals to assist you in providing comprehensive service to your patients.
