We look forward to helping you get back in the game and back to your life.
Our first appointment will require the usual timely paperwork. For new patients, we recommend an early arrival of at least a half hour before your appointment time.
In order to save some time on your first visit, we have a quick checklist of items our new patients will need upon arrival and check in. We’ve also provided a few short-cuts by having many of the forms available for filling out or review, prior to the appointment.
If you have any questions when filling out these forms please fill-in what you can, or if any additional questions, save them for the day of your visit and our staff will assist in answering.

These forms are available here to print, fill out, and bring to the patients first appointment. If the
patient doesn’t have a chance to fill them out prior to the appointment, please arrive early and allow
extra time to complete the forms.

can be printed and brought to the appointment, make sure they are not signed. They need to be
witnessed by a Major Sports Medicine staff member.
