Appointment Request
If you are new to us, or needing a return appointment, please use our convenient online Appointment Request form, or give us a call at (317) 392-2161 during normal business hours. If all lines are busy, please leave a message and contact number on the answering machine and someone will return your call at their earliest convenience.

Medical Records Request
If you are able to bring records from your physician, this would be very helpful. If not, we have forms you can sign to request those records. Standard recovery and processing fees do apply. Questions or concerns, please give us a call at (317) 392-2161.

We do fill out Family Medical Leave Act (FMLA) forms and, disability forms. There is a $40 charge for processing fee.

Prescription Renewal Process
If you need to refill on your medication, please contact our office no less than 24 hours before your prescription has run out. We require at least 24 hours (excluding weekends) for prescription request turn-around time.

Prescriptions are not filled after hours or over the weekend, therefore patients are encouraged to call Monday – Friday during business hours for a prescription request.

Our trained medical staff will review the request, and once approved by the physician, we will call in the refill to your pharmacy of choice. You will be notified that your refill has been phoned in.
